Includes permanent housing through the Department of Health Services (DHS) and the Housing for Health (HFH) programs. We provide a participant-centered approach sensitive to the challenges of those experiencing homelessness. We help with a wide range of social, logistical, medical, and behavioral health issues individuals face as they move into and maintain permanent supportive housing. We employ a whatever-it-takes approach to assist participants in their transition from homelessness to permanent housing.
