Provides assistance for justice affected men and women who need referrals, direct services, and other resources. Community Health Workers with lived experience help with a wide range of social, logistical, medical, and behavioral health issues that this population faces as they transition back into the community. We employ a whatever-it-takes approach to assist all of our participants.
